TRANSFORM LDL-C Risk

LDL Management in the High-Risk Patient

The ACC is excited to announce a new initiative to identify high-risk patients with a history of cardiovascular events related to atherosclerotic cardiovascular disease (ASCVD) and to optimize their access to the latest evidence-based treatments for LDL-C lowering and cardiovascular event risk reduction.

TRANSFORM LDL-C Risk leverages the ACC’s PINNACLE Registry, cardiology’s largest outpatient clinical data registry, to identify secondary prevention ASCVD patients in select regions of the U.S that meet the evidence-based criteria for enhanced treatments.

This initiative will involve a collaborative, regional network of practices that can share best practices and influence future ACC work in this space. It is the first of several planned TRANSFORM initiatives to turn ACC clinical algorithms and standards into actionable results for patients, while lowering the administrative barriers for clinicians and patients.

This initiative is a part of multi-project series developed by the ACC using data analytics, ACC clinical algorithms, and the PINNACLE Registry to focus and TRANSFORM the delivery of care for patients.

TRANSFORM LDL-C Risk is funded by Sanofi and Regeneron in partnership with the American College of Cardiology.
