Choose the Accreditation Path That Fits Your Program
Heart Failure Accreditation offers two designation options based on your organization's capabilities and approach to heart failure care.
- HF: Best for facilities focused on inpatient heart failure management and established referral pathways.
- HF with Outpatient Services: Best for facilities providing longitudinal heart failure management in partnership with an outpatient facility. This designation is required for Heart Failure Partner in Care participation.
