Heart Failure Partner in Care
The Heart Failure (HF) Partner in Care program recognizes outpatient providers that maintain a strong, collaborative partnership with an ACC HF Accredited facility.
Eligibility
- The outpatient provider must be in partnership with facilities applying for HF Accreditation with the Outpatient Services Designation.
- Partnering facility can demonstrate outpatient partnership by submitting supporting documentation of this collaboration to meet the "Outpatient Services" Essential Components.
How It Works
- The partnering facility lists the outpatient provider(s) in their application to the HF Accreditation tool.
- ACC Accreditation Services validates supporting documentation provided and notifies the facility of the approved recognition.
- The facility must notify the outpatient provider(s) of the approved recognition.
- ACC Accreditation Services send the facility a marketing kit that includes.
What You Receive with Recognition
Earning HF Partner in Care recognition signals a strong, coordinated partnership between the facility and outpatient providers, reinforcing confidence for patients and the community. Once awarded, the facility and outpatient provider(s) will be given:
- A marketing kit that includes press release statements, social media graphics and messaging.
- ACC Accreditation Services promotes the recognition through ACC's communication channels and the ACC's annual ad insertion in the U.S. News & World Report "Best Hospitals"edition.
Take The Next Step
Facilities can get started by listing the outpatient provider(s) in their application in Essential Component (EC) EC9.M4 in the 'Comments' tab within their Accreditation tool.
