
Actionable PINNACLE Insights – LDL Management in the High-Risk Patient
Utilizing the PINNACLE Registry, this innovative program is aimed to help reduce heart attacks and death in very high-risk patients who have had a prior cardiovascular event. Specifically, this project addresses patients with dyslipidemia that are eligible for treatment intensification and managing these patients through the clinical pathway lipid goal (2016 ACC Expert Consensus Decision Pathway on the Role of Non-Statin Therapies for LDL-Cholesterol Lowering in the Management of Atherosclerotic Cardiovascular Disease Risk). Leveraging patient characteristics within PINNACLE, your practice will receive a list of patients who are at risk of an event in the next 3-5 years and have an immediate treatment opportunity based on the pathway.
Through this initiative, your practice will receive:
- cohort specific reports that provide actionable patient lists containing 5-100 very high-risk patients;
- patient and physician stratification reports to understand opportunities for partnership with colleagues and patients;
- access to an accelerated payer approval process for patients fitting objective eligibility criteria;
- enhanced relationships with pharmacy benefit managers providing increased transparency and insight into prescription use and adherence;
- medication management plans to optimize the efficacy of clinician and patient actions to initiate, titrate, and adhere to therapy based on the pathway;
- access to an ACC quality ambassador and innovative regional learning collaboratives;
- opportunities to partner with regional employer/purchaser coalitions to demonstrate value; and
- medium-weighted credit in the MIPS Performance Category "Improvement Activities".
This project involves a collaborative regional network of practices that can share best practices and influence future ACC work at the national level in this space. It is the first of several planned initiatives to turn ACC standards into actionable results for patients, while lowering the administrative burden for clinicians.
Throughout this initiative, sites will have bi-weekly conference calls with ACC Quality Ambassadors and to discuss progress through the patient lists, address gaps, and collect best practices.
