Patient Navigator Program Focus MI

The American College of Cardiology (ACC) is excited to launch the next generation of the Patient Navigator Program, called the ACC Patient Navigator Program Focus MI.  This will be a national program specifically designed to enhance the care and outcomes for myocardial infarction patients.  Hospitals that participate in the ACC's NCDR® ACTION Registry® can opt into the program starting September 21
Note: You will be required to log in using your NCDR user name and password to opt into the program.

Upon opting into the program, you will have access to national benchmarks, with comparison data at your fingertips.  Resources available to participating hospitals have been tested over the past two years by our flagship Navigator Hospitals.
