Diplomat Hospitals are a key component of the Patient Navigator Program: Focus MI. Fifteen hospitals that participated in the first phase of the Patient Navigator Program focused on reducing 90-day readmissions for MI patients and providing mentorship for wider adoption of the program across other institutions. With 90-day readmissions an increasing area of focus for proposed payment and reimbursement models, these hospitals were on the cutting edge of identifying best practices.
