Listserv

The H2H Listserv is a network of individuals committed to improving transitions of care. Through the H2H Listserv, you can send and receive messages directly to your email inbox. To prevent an overflow of email messages and to track responses, the H2H Listserv is moderated. An average of 20 messages are sent and released each week depending on the topic of discussion. Listserv messages are released in the evenings several days a week.
